29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) On or about March 7, 2023, and at times prior thereto, a program to include procedures, training, and inspections had not been established for employees performing service and maintenance activities. An employee was exposed to serious injury when performing maintenance under an elevated Toro Groundmaster 4500-D/4700-D, Model 30857, S/N 313000230 industrial lawn mower. Abatement certification AND documentation are required for this item.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about March 7, 2023, and at times prior thereto, specific energy control procedures were not established by the employer prior to employees performing activities including but not limited to, repairs and maintenance for industrial lawn mowers in the maintenance shop. Hazardous energy release due to unexpected motion was not prevented prior to maintenance of a Toro Groundmaster 4500-D/4700-D, Model 30857, S/N 313000230 industrial lawn mower. Abatement certification AND documentation are required for this item.
