29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Worksite located near 10709 Hwy BB, Kearney, Missouri- Employee was exposed to a struck by hazard and was not wearing a hard hat while working adjacent to the excavation equipment's arm, which was swinging as it was digging and he was attempting to place a box in the ground.

29 CFR  1926.651(d):Employees exposed to public vehicular traffic were not wearing warning vests provided by the employer:  Worksite located near 10709 Hwy BB, Kearney, Missouri- Employees who were exposed to vehicular traffic were not wearing warning vests needed to make themselves  as visible as possible to oncoming vehicular traffic.
