29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  The Beckman & Gast Co. - St. Henry, Ohio: On or about March 28, 2023, the employer did not develop and document energy control procedures for machine such as, but not limited to, the east and west tomato peeler and sorter tables.  Employees that setup and maintain the machines were exposed to the equipment's unexpected startup.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  The Beckman & Gast Co. - St. Henry, Ohio: On or about March 28, 2023, the employer did not ensure that employees who service and maintain equipment such as, but not limited to, that the east and west tomato peeler and sorter tables were trained in the skills required for the safe application, usage, and removal of energy controls. Employees that service the equipment not trained to isolate and/or render the electrical/mechanical energy source safe.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  The Beckman & Gast Co. - St. Henry, Ohio: On or about March 28, 2023, the employer did not ensure that the east and west tomato peeler and sorter tables were shut down and energy control steps were implemented by each authorized employee to control the mechanical energy of the equipment's rotating, and chain and sprocket parts.  Employees who performed servicing tasks were exposed to the unexpected startup of the equipment.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employees who operated the equipment and loaded labels were exposed to injury from the unguarded rotating parts and nip points.
