29 CFR 1910.147(c)(4)(ii): The lockout/tagout procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures:  a) 151 Industrial Drive Frankfort, NY 13340, FANUC ROBODRILL Serial No. P991TH427, on or about 3/29/23:  The written lockout/tagout procedures were not adequate for equipment, including, but not limited to the  FANUC ROBODRILL, in that they did not clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy, and it did not identify all energy sources, including, but not limited to, pneumatic sources of energy.  b) 151 Industrial Drive Frankfort, NY 13340, Genos N460V-5AX-Okuma, on or about 3/29/23:  The written lockout/tagout procedures were not adequate for equipment, including, but not limited to the Genos N460V-5AX-Okuma, in that they did not clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy, and it did not identify all energy sources, including, but not limited to, pneumatic sources of energy.  Abatement certification is required.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and requirements of this standard were followed:  a) 151 Industrial Drive Frankfort, NY 13340, FANUC ROBODRILL Serial No. P991TH427, on or about 3/29/23:  Annual periodic inspection of the lockout/tagout procedures were not being conducted to ensure that the procedures and requirements of the standard were being followed. FANUC ROBODRILL Serial No. P991TH427 lockout tagout procedure was established in 2005 and no inspection procedures were provided upon request.  b) 151 Industrial Drive Frankfort, NY 13340, Saw Horizontal Band W.F. Wells & Sons, on or about 3/29/23:  Annual periodic inspection of the lockout/tagout procedures were not being conducted to ensure that the procedures and requirements of the standard were being followed.  Saw Horizontal Band W.F. Wells & Sons was established in 2010 and no inspection procedures were provided upon request.  Abatement certification is required.
