29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.  On or about March 29, 2023, in the machine shop, the employer exposed machinist employees to amputation hazards in that the 55 Ton Ironworker did not have a punch guard at the point of operation.  b.    On or about March 29, 2023, in the machine shop,  the employer exposed machinist employees to an amputation hazard in that the Turn-N-Bend press foot actuator pedal, that is integral to guarding the point of operation, was missing the pedal cover intended to prevent unintended operation.
