A. On, or about, April 19, 2023, an employee who was assigned to the work in the COVID-19 unit had not received a fit test for the N95 respirator that was being worn. B. On, or about, December 26, 2022, an employee who was assigned to work in the COVID-19 unit had not received a fit test for N95 respirator that was worn. Also, the previous documented fit test had been conducted on June 29, 2020 for a respirator that was no longer provided. C. On, or about, December 26, 2022, an employee who was assigned to work in the COVID-19 unit had not received a fit test for the N95 respirator that was being worn. D. On, or about, April 1, 2023, an employee who was assigned to work in the COVID-19 unit had not received a fit test for N95 respirator that was worn.
