29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means shall be fully lowered, controls shall be neutralized, power shall be shut off, and brakes set. Wheels shall be blocked if the truck is parked on an incline.  a. On or about March 21, 2023, and at times prior, the employer did not ensure that powered industrial vehicles (PIV) were powered off when left unattended in the warehouse. On March 21, 2023, an employee was struck by a Raymond, Model 8400, Serial 840-08-76171, electric rider pallet jack when the lift moved unexpectedly.
