29 CFR  1910.138(a): General requirements. The employer did not select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes.   On or about March 29, 2023, employees were exposed to burn hazards while filling the glue pot with glue pellets. The glue pot has a small red handle employees can use to open the top to dump the pellets in using a metal scooper. The red handle does not get hot but is sitting on top of a metal container which gets hot. The glue melts to a temperature of approximately 250 degrees Fahrenheit. Employees were not required to wear heat resistant gloves while performing this task.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence:   (1) prepare for shutdown, (2) shut down machine or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device by authorized employee, (5) relieve, disconnect, restrain or otherwise render safe all stored or residual energy, (6) verify that isolation and deenergization of the machine or equipment have been accomplished:  On or about March 28, 2023, employees working on line 2 were exposed to puncture and amputation hazards when servicing bailer 4. Employees would grab a pair of pliers tied to the bailer to remove stuck pieces of wire without using lockout/tagout.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.212(a)(1): The employer did not ensure one or more methods of machine guarding were provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about March 28, 2023, employees were exposed to puncture and amputation hazards from bailer 4 on line 2 due to the lack of a machine guard. The bailer punches holes in paint cans and attaches a metal wire to serve as a handle. There was no means of guarding on the bailer. The bailer was missing a door on both the front and back side, exposing employees to amputation  and puncture hazards, and flying pieces of metal.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
