29 CFR 1926.150(c)(1)(vi):A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite. a) Willamette River Morrison Street Bridge, Portland, Oregon on or about 29 March 2023 and at times prior thereto, Employees were exposed to a fire, burn hazard from a class IV flammable storage tank that did have a fire extinguisher, rated not less than 10B within 50 Feet of the 500-gallon storage tank being used on the jobsite.

29 CFR 1926.150(c)(1)(viii):Portable fire extinguishers were not inspected periodically and maintained in accordance with maintenance and use of portable fire extinguishers, N.F.P.A. No. 10A-1970: a) Willamette River Morrison Street Bridge, floating barge worksite, Portland, Oregon on or about 29 March 2023 and at times prior thereto, Employer exposed employees to a fire, burn hazard by, not maintaining portable fire extinguisher in accordance with maintenance and use of portable fire extinguishers, N.F.P.A. No. 10A-1970. A discharged portable fire extinguisher which was stored in the mil van on the barge worksite.
