29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   a) 1027 West College Avenue, York PA, Front Porch Roof, - Employees did not don safety glasses while using pneumatic nail guns to attach shingles to the roof of a townhome building, on or about  March  29, 2023.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 1027 West College Avenue, York PA, Roof, Fall from Second Story - Employees performed residential roofing activities on a two-story residential roof,  approximately 20 feet above grade with an 4/12 roof pitch, without utilizing any means of fall protection, on or about March 29, 2023.  b) 1027 West College Avenue, York PA, Front Porch Roof - Employees performed residential roofing activities on a one-story porch roof,  over six feet above grade with an 4/12 roof pitch, without utilizing any means of fall protection, on or about March 29, 2023.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 1027 West College Avenue, York PA, Front Porch Roof -  Employees were utilizing an extension ladder that did not have its side rails extended at least three feet above the upper landing surface, on or about March 29, 2023.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
