29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Jobsite, 722 Ridge Road, Nicktown, PA 15762: On or about March 17, 2023, a 12-ton capacity Hydra-Power press brake (model number: 16-36, Serial Number: 6200177109) was not turned off or shut down using the procedures established for the machine or equipment. An employee received injuries to his left hand while changing a die on the press brake while the machine was not turned off or shut down using procedures established for the machine or equipment.
