At a temporary residential site located at Building 7, 2nd floor - Breezeway, at 459 Wild Light Avenue, Yulee, FL 32097: On or about March 28, 2023, the employer exposed employees to fall hazards, in that the breezeway did not have the flooring or midrail up, exposing employees to 9 foot fall hazards.

At a temporary residential site located at 459 Wild Light Avenue - Building 7, Yulee, Florid 32097. On or about March 28, 2023, on a 6:12 pitched roof, the employer exposed two employees to fall hazards ranging from 30 to 38 feet 4-inches, in that fall protection was not utilized by the employees, while bracing the trusses and decking the roof.
