29 CFR 1926.304(d):Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: On or about March 30, 2023 at approximately 1150hrs, on a residential construction site in vicintiy of 212 Redbarn Ln Caddo Mills, TX. Two employees were observed operating DeWalt 60v circular saws with modified guards exposing employees to point of operation machine guarding hazard.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about March 30, 2023 employees were exposed to injury from fall while working at height of greater than 6 feet above lower level without fall protection. The employer, Joel Rangel Mendoza, was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1388920, citation number 1, item number 3a and was affirmed as a final order on January 16, 2020, with respect to a workplace located at 4213 Blythe St. Rockwall, TX 75032.
