Location: Worksite  On or about March 31, 2023 and at times prior, workers were exposed to potential fire hazards while working were scrap lumber was not removed from the immediate work area as the work progressed.

Location: Worksite  On or about March 31, 2023 and at times prior, employees were exposed to overhead struck-by hazards while working where an enclosed waste shoot was not used to to remove construction debris dropped more than 20 feet to points lying outside the exterior walls of the building.
