29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about March 31, 2023, at a residential construction site located on 1018 Newport Creek Dr. in Katy, TX, an employee was exposed to a fall hazard when using an extension ladder to access the roof without the side rails at least three feet above the roof landing.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  On or about March 31, 2023, at a residential construction site located on 1018 Newport Creek Dr. in Katy, TX, an employee was exposed to eye injuries while operating pneumatic nail guns without wearing eye protective equipment.  ALFREDO MEZA DE LA CRUZ, WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.102(a)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1580991, CITATION NUMBER 1, ITEM 1 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 14, 2022, WITH RESPECT TO A WORKPLACE LOCATED AT 7323 KINGBIRD COVE CT. IN KATY, TX 77493.

On or about March 31, 2023, at a residential construction site located on 1018 Newport Creek Dr. in Katy, TX, an employee was exposed to a fall hazard greater than 6 feet while performing roofing work on the roof of a two-story house without the use a fall protection system.
