Employees were exposed to serious injuries in that a written hazard assessment certification had not been completed to certify the personal protective equipment needs and requirements for the employees performing electrical work on a Siemens JXD63B300 300A 600 Volt 3 Phase electrical panel rated 600 maximal Volts.

Employees were exposed to shock and electrocution hazards when working on an energized Siemens JXD63B300 300A 600 Volt 3 Phase electrical panel rated 600 maximal Volts. The employer did not ensure that safety-related work practices were employed when work was performed near or on equipment or circuits which were or could be energized.

Employees were exposed to shock and electrocution hazards when working on an energized Siemens JXD63B300 300A 600 Volt 3 Phase electrical panel rated 600 maximal Volts. The employer did not ensure employees working on energized equipment did not disconnect circuits and equipment to be worked on from all energy sources prior to performing work.

At a worksite located at 1313 Riley Industrial Dr, Moberly, Missouri. Employees were exposed to shock and electrocution hazards. Unqualified maintenance workers were rewiring an energized Siemens JXD63B300 300A 600 Volt 3 Phase electrical panel rated 600 maximal Volts. The employer did not ensure that workers were capable of working safely on energized circuits or familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools.
