29 CFR  1926.501 (b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) Residential roof structure, various locations; Employees engaged in roofing construction activities from a roof located on an under-construction multiplex, were not protected from falls while working at approximate heights ranging between 19-feet, 3-inches to 27-feet, 4-inches above ground level.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) West side of residential structure; A Keller brand multi-position telescopic aluminum ladder placed at ground level and used as an extension ladder by workers to access and egress the roof, was not adjusted to allow for the side rails to extend at least 3 feet above the landing surface, was not equipped with a grasping device(s) and was not secured to prevent displacement.
