29 CFR  1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  Location: 3193 Kennedy Blvd, Jersey City, NJ 07306  a) The employer failed to provide abatement certification and documentation demonstrating that abatement was complete for Citation 1 Item 1 (1926.21(b)(2)) which was issued on 9/27/2023, was received on 9/29/2023, and became final order on 10/24/2023.  Violation occurred on 10/20/23.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   a) 3193 Kennedy Blvd., Jersey City, NJ  Employees engaged in demolition activities were removing materials and debris during the demolition of a residential dwelling were not provided with information and training to recognize potential hazards or unsafe conditions while performing such job duties, which exposed them to fall hazards, on or about March 27, 2023.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.)  3193 Kennedy Blvd., Jersey City, NJ  The employer did not ensure that employees were protected from fall hazards.  Employees were engaged in residential demolition activities, working at or near open floor holes in several locations on the second level, and were not utilizing personal fall arrest systems or protected by any other means of fall protection, which exposed them to fall hazards, on or about March 27, 2023.
