29 CFR 1910.25(b)(5)(i): The employer did not ensure that when a door or a gate opens directly on a stairway, a platform is provided, and the swing of the door or gate does not reduce the platform's effective usable depth to less than 20 inches (51 cm) for platforms installed before January 17, 2017. On or about April 11, 2023, employees were exposed to fall hazards when a door for noise barrier enclosure on a shaver platform opened directly on to a stairway and did not have a platform.

29 CFR 1910.95(c)(1):The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: In the following instances, employees were exposed to noise levels exceeding an 8-hour time weighted average of 85 decibels measured on the A scale, or equivalently a dose of fifty percent, and the employer did not have a hearing conservation program: a. A log roller on line 1 was exposed to continuous noise at 373.3% of the daily noise exposure (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of approximately 99.5 dBA during the 454 minute sampling period on April 11, 2023; exposure calculations included a zero increment for the 26 minutes not sampled. b. A log roller on line 2 was exposed to continuous noise at 415.6% of the daily noise exposure (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of approximately 100.2 dBA during the 457 minute sampling period on April 11, 2023; exposure calculations included a zero increment for the 23 minutes not sampled.
