On or about April 3, 2023, an employee was exposed to struck-by hazards while surfacing metal beam columns utilizing a 5" Fixed Speed Angle Grinder with Paddle Switch without the safety guard.

29 CFR 1926.350(a)(9): Compressed gas cylinders shall be secured in an upright position at all times except, if necessary, for short periods of time while cylinders are actually being hoisted or carried.  On or about April 3, 2023, employees were exposed to struck-by due to the employer did not ensuring that the oxygen compressed gas cylinder (UN 1072/CAS 7782-44-7) on location was secured.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about April 3, 2023, five employees engaged in commercial construction were exposed to fall hazards in excess of 6-feet without using any form of fall protection. The employees were observed on edge of the first-floor roof of a two-story hotel being constructed.
