29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about April 3, 2023, employees were exposed to fall hazards because the 24-foot Werner ladder they were using did not extend at least 3 feet (.9 m) above the upper landing surface (rooftop 18-feet high) to which the ladder was used to gain access.
