On or about April 03, 2023, during a construction project on the roof of a commercial building located at 29000 Aurora Rd, Solon, OH 44139, the employer did not ensure employees working from a low-sloped roof were protected from a fall of approximately 20.1 feet by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system.
