29 CFR 1910.178(c)(2)(iv): Power-operated industrial trucks designated as DY, EE, or EX were not used in locations where volatile flammable liquids or flammable gases were handled, processed or used, but in which the hazardous liquids, vapors or gases were normally confined within closed containers or closed systems from which they could escape only in case of accidental rupture or breakdown of such containers or systems, or in the case of abnormal operation of equipment: Warehouse: On, or about, April 3, 2023, E-rated forklift trucks were used to transport 55 gallon drums of Methyl ethyl ketone (MEK), a Category 2 flammable liquid, to and from the reactor rooms and to place and retrieve the drums from the racking system. In the event of a spill or rupture of a drum of MEK, the forklift trucks would be a potential ignition source.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: Warehouse, Aisle D: On, or about, April 3, 2023, barrels that were being placed onto or retrieved from the storage racks came within inches of a live, 480 volt buss bar. The buss bar enclosure was not protected from physical damage.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910: Warehouse, Aisle D: On, or about, April 3, 2023, a barrier was installed in front of two General Electric Load Centers, Panels P and K. The barrier was located in the working space for the panels.
