Toledo OH 43614: The employer did not ensure employees were protected from falls while performing roofing work (installing shingles) atop a two-story residential structure. Employees operating in this conditions were exposed to falls of 22 feet to the ground below.

29 CFR 1926.1053(b)(1):Portable ladder was used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a rigid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder: a) Anthony Cox, on or about April 3, 2023 at the address of 3551 River Rd. Toledo OH 43614: The employer did not ensure employees using an aluminum ladder had the side rails extended at least three feet above the landing surface. Two employees performing roofing work activities were exposed to fall hazards while using ladder to access/exit roof on a two-story residential structure at a height of 22 feet from ground below.
