On 3/27/23, an employee sustained an right hand injury while cleaning out the ribbon blender at the unguarded point of operation. b) Powder Lab: Ribbon Blender: Ribbon blender did not have the point of operation guarded to prevent employees exposure to an amputation/laceration hazard from the unguarded ram on or about 4/3/23. The Cosmax USA Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard ), which was contained in OSHA 29 CFR 1910.212(a)(3)(ii) inspection number 1469646, citation number 1, item number 1and was affirmed as a final order on (5/21/20), with respect to a workplace located at 3366Miller Park Dr. Garland TX 75042.
