On the roof of a temporary residential worksite, located at 18 College Street, Jacksonville, FL 32205: On or about April 4, 2023, the employer exposed its employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using pneumatic nail guns to install decking.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   a.  On a residential worksite,  located at 18 College Street, Jacksonville, FL 32205:  On or about April 4, 2023, and at times prior, the employer exposed its employees to fall hazards of 12 feet 5-inches, in that fall protection was not utilized by employees while installing decking.
