29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.   a) On or about March 29, 2023, the Polytruder 6-28 puller on line 1 did not have guarding to protect the operator from accessing the pinch point. This exposed the operator to amputation hazards.  b) On or about April 5, 2023, the Automated Manufacturing Systems 6-30 A1000 puller on line 1 did not have guarding to protect the operator from accessing the pinch point. This exposed the operator to amputation hazards.  c)  On or about April 5, 2023, the Conair 6-39 puller on line 4 did not have proper guarding to protect the operator from accessing the pinch point. This exposed the operator to amputation hazards.

29 CFR  1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a) On or about April 5, 2023, at the workplace located at 5405 South Westridge Court New Berlin, WI 53151, the employer did not establish a program consisting of machine specific energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative for employees that were engaged in service and maintenance activities on equipment, such as, but not limited to the extruder, puller, and grinder and  where the unexpected energizing, start up or release of stored energy could occur and cause injury.

29 CFR  1910.147(c)(7)(i):The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:  A. Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) On or about April 5, 2023, at the workplace located at 5405 South Westridge Court New Berlin, WI 53151, the employer did not establish a program consisting of  employee training to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative for employees that were engaged in service and maintenance activities on equipment, such as, but not limited to the extruder, puller, and grinder and  where the unexpected energizing, start up or release of stored energy could occur and cause injury.
