The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided. a.) Goodwill Titusville PA ; back storeroom - On or about 4/5/23, and at times prior thereto, during and after periods of rain, water leaks from the ceiling and from the exterior sidewalk under the Donation Door into the work areas where employees are located. A floor drain, or similar, is not available in the area. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Goodwill Titusville PA ; back store room - On or about 4/5/23, and at times prior thereto, the microwave, refrigerator and time clock were attached to a surge protector. The surge protector was connected to a flexible extension cord. The extension cord was run to the other side of the room and plugged in. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

The cable assembly was not protected by devices such as rubber sleeves, grommets, and/or conduit to prevent physical damage such as rubbing and chafing.
