29 CFR 1926.1203(a): Before work begins, the employer did not ensure that a competent person identifies all confined spaces in which one or more of the employees are to enter through consideration and evaluation of the elements of that space, including testing as necessary: a. Water Service Detail manhole NE side of project: On or about March 28, 2023, the employer exposed two employees to confined space hazards in that the employer allowed employees to enter a manhole confined space inside a newly constructed water/sewer line without first evaluating the space to determine if it was a permit required confined space.
