(a)	Second Floor Warehouse Area, 920 Irwin Run Road, West Mifflin, PA: On or about March 30, 2023, the employer did not ensure that each employee working on the second, third, and fourth levels of the mezzanine were protected from falling through the use of guardrail systems, safety net systems, or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. The employees were exposed to an approximate 12 foot fall hazard to the concrete floor below while working on the second level of the warehouse racking system. Employees would transverse on this level in order to store goods and also retrieve the goods that were being stored for shipment.
