29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: On or about April 6, 2023, at the CNC machine and the Fiberglass Roll Room warehouse, the employer did not have a written respiratory protection program with all of the elements required to protect the health of employees who were provided and required to use HDX N95 Respirator Masks while cleaning and rolling fiberglass material. Employees used chemical products such as, but not limited to DCPD Laminating Resin, NOROX MEKP-925, and APG 1750 S Orange Resin, containing chemicals, including but not limited to butanone, carbon black, and styrene.

Employees were provided and required to use HDX N95 Respirator Masks, without being provided a medical evaluation.
