29 CFR 1910.178(m)(5)(iii): When the operator of an industrial truck is dismounted and within 25 ft. of the truck still in his view, the load engaging means shall be fully lowered and the brakes set to prevent movement. a) Production Area - On or about March 30, 2023, at the reliant unwinder machine, the employer failed to ensure a material handler operating an electric Clark forklift fully lowered the load and set the brakes prior to dismounting the forklift to adjust the adjacent staging table. The affected employee was thereby exposed to a struck-by hazard when the master roll of "board" being transported on the forklift rolled off of the forks and struck the employee resulting in a broken left leg.
