29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   On or about April 6, 2023, the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections.  Employees using the following machines to include but not limit to the DMX Quad Acction 200 HD, Biro 1056 A-20, Talsa K80V, Stuffer Reiser, Gerta 420 Pack 3000/Vesta 420, Rollstock Model RA 200S, and the XTRAVAC860A8 ,  were not protected from the unexpected energization, startup, or release of stored energy while performing servicing and maintenance activities, such as, but not limited to, clearing jams, cleaning and sanitizing, and mainteanace activities. Energy sources include, but not limited to, electrical and pneumatic energy.
