On or about March 22, 2023, in Cumene Plant at INEOS Phenol located in Pasadena, TX, workers were exposed to fire and explosion hazards, when the employer did not ensure that the employees (drivers) were present close to the connections during the transfer of LPG/propylene from tank trucks to the facility (owners) process piping.

On or about March 22, 2023, in Cumene Plant at INEOS Phenol located in Pasadena, TX, workers were exposed to fire and explosion hazards, when the contract employer did not ensure that the LPG/propylene tank truck drivers have knowledge and training of procedures that were being used for loading /unloading activities.
