On or about April 6, 2023, Employees performing roofing activities were exposed to fall hazards on a construction site in Paxton, Illinois, and the employer did not ensure an adequate fall protection system was being used. TORNADO ROOFING, INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1551986, CITATION NUMBER 2, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 5, 2021, WITH RESPECT TO A WORKPLACE LOCATED AT 4110 RAYBURN CT, CHAMPAIGN, IL 61822
