29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:  a) On or about April 6, 2023, and at times prior, employees were exposed to eye and face hazards while dispensing propane into portable or recreational vehicle (RV) tanks from bulk tank onsite.  This exposed employees to severe and possibly permanent eye damage from freeze burns.  b) On or about April 6, 2023, and at times prior, employees were exposed to eye and face hazards while watering golf cart batteries. This exposed employees to severe and possibly permanent eye damage from corrosive chemical burns.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:  a) On of before April 6, 2023, and at times prior, employees were exposed to liquid propane and the employer did not require employees use appropriate hand protection. They were provided grey mesh glove, with blue foam dipped palm for grip and were not insulated and the employer did not require them to be used.  This exposed employees to direct contact with the liquefied gas can chill or freeze the skin (frostbite).

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2):  a) On or about April 06, 2023, and at times prior, employee deenergized an outlet above 50 volts to replace GFCI outlet were not protected from electrical shock and the circuit energizing the outlet was not locked or tagged out during replacement of the GFCI outlet.  this exposed the employee to shock hazards.
