29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 06, 2023, during a construction project of a residential home located at 36414 Lakeshore Blvd, Eastlake, OH 44095, the employer did not ensure that the portable ladder that employees used to gain access to the roof, extended 3 feet above the roof (upper landing surface), exposing employees to falls hazards of approximately 11.8 feet while climbing the ladder to access the roof.
