29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  On or about April 6, 2023, and times prior thereto, a four way outlet box attached to a maintenance cart was plugged into the wall four way outlet where power cords from the cart outlet energized small maintenance tools next to welding cylinders, exposing employees to fire and explosion hazards.

29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.   On or about April 6, 2023, and times prior thereto, at the B side production office, a fire extinguisher was not identified with a fire extinguisher sign and readily accessible.  The employees were exposed to fire and smoke hazards.

29 CFR  1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):   On or about April 6, 2023, and times prior thereto, on the west side of the tank farm area, an employee connected an extension cord with the missing ground pin to a maintenance cart outlet box to energize small maintenance tools, exposing employees to electrical hazards.
