29 CFR  1910.28(b)(8):29 CFR  1910.28(b)(8): Where there were repair pits, service pits, and assembly pits less than 10 feet in depth, the employer did not limit access within 6 feet (1.8 m) of the edge of the pit to authorized employees trained in accordance with � 1910.30; place floor markings at least 6 feet (1.8 m) from the edge of the pit in colors that contrast with the surrounding area; or provide a warning line at least 6 feet (1.8 m) from the edge of the pit with stanchions capable of resisting, without tipping over, a force of at least 16 pounds (71 N) applied horizontally against the stanchion at a height of 30 inches (76 cm); or provide a combination of floor markings and warning lines at least 6 feet (1.8 m) from the edge of the pit.  Location: Repair Bay - Northwest side of facility. On or about April 6, 2023 the employer did not limit access, place floor markings, or provide a warning line around the repair pit used for servicing vehicles.

29 CFR  1904.41(a)(2): The establishment had 20 or more but fewer than 250 employees at any time during the previous calendar year, and the establishment was classified as an industry listed in appendix A to subpart E of this part, but did not electronically submit information from OSHA 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee.  The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  McKeon and McKeon dba Joshua's Worldwide, 172 Stoddards Wharf Road Gales Ferry, CT 06335:  On or about April 6, 2023, and times prior thereto, the employer failed to electronically submit information from their OSHA Form 300A Summary of Work-Related Injuries and Illnesses for 2022 or equivalent on or before 03/02/2023.
