29 CFR 1910.22(a)(3):29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. a) On or before April 05, 2023, the dock plate used to access parked semi-trailers was damaged and protruded up approximately 2.5 inches exposing employees to a tripping/fall hazard.

29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) On or before April 05, 2023, an employee was directed to operate power industrial trucks in the workplace but was not trained and certified exposing employees to a crush/struck by hazards. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(1)(i), which was contained in OSHA inspection number 1455314, citation number 1, item number 1 and was affirmed as a final order on 03-03-2020, with respect to a workplace located at 1150 N. McDowell Blvd, Petaluma, CA 94999. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(1)(i), which was contained in OSHA inspection number 1368043, citation number 1, item number 1 and was affirmed as a final order on 03-08-2019, with respect to a workplace located at 7451 Templehof Drive, Indianapolis, IN 46241.
