29 CFR  1926.102(a)(1):Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about April 6, 2023, during the installation of framing material at the job site located at 9180 Ledgeview TR, Broadview Heights Ohio 44147; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about April 6, 2023, during the installation of framing material at a job site located at 9180 Ledgeview TR, Broadview Heights Ohio 44147; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 19.8 feet by utilizing a fall protection system.
