On or about April 5, 2023, the employer did not ensure that listed or labeled equipment was installed and used in accordance with any instructions included in the listing and labeling in the following instances: 1: An orange extension cord was connected in series to a yellow extension cord and routed to a 4 outlet to power a honey pot. 2: A power tap designed by manufacturer for indoor use was connected to honey pot located outside crew house trailer # 328B. 3: A yellow extension cord was connected in series to an indoor power tap and used as a power source for a grill located outside trailer.

Missing cover on an outlet located on the outside of Bunk House # 1 2. Missing cover on an outlet located on the outside of Bunk House # 2 Unit Drilling Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.303(e)(1), which was contained in OSHA inspection number 1625229, citation number 2, item number 1 and was affirmed as a final order on 02-27-2023 , with respect to a workplace located at Montrose 45 Unit, Well 7H (31�51'53.3"N 104�09'59.8"W), 15.4 Miles NW of Orla .

This exposed employees to electrical shock hazards in the following instances: 1: 240-volt electrical box positioned on the outside of bunk house # 1. 2: 240-volt electrical box positioned on the outside of bunk house # 2. 3: Electrical box located left at the entrance of the change house.
