Except as provided in paragraphs (a)(2), (a)(3), (a)(4), (a)(5) and (g) of this section, each scaffold and scaffold component shall be capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it.   a) On April 6, 2023, located at the exterior east side of a commercial building, the employer failed to protect employees climbing the guardrails of a JLG ES1932 Scissor Lift, serial number 11113937, in that the machine was placed in an unstable configuration during use. The machine was not designed to handle the loads imposed by employees climbing onto and standing on the guardrails of the scissor lift, changing the rated capacity of the machine and the required 4:1 safety factor. As a result, employees were exposed to a fall hazard of 15 feet above ground level.
