29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  On or about April 7, 2023:  a. In the southeast area of the shop, an employee was exposed to a struck-by hazard when operating a Hypertherm (brand), Jet Liner II waterjet cutting system, without point of operation guard.  b. In the southeast area of the shop, an employee was exposed to a struck-by hazard when operating a Hypertherm (brand), Jet Liner II waterjet cutting system, without point of operation guard.  c. In the center of the shop, an employee was exposed to a struck-by hazard when operating an Accupress (brand), press brake, without point of operation guard.  d. In the southeast area of the shop, an employee was exposed to a struck-by hazard when operating a Duro Dyne (brand), Mach II Pinspotter, without point of operation guard.  e. In the southeast area of the shop, an employee was exposed to a struck-by hazard when operating a GripNail (brand), Power Pinner- 7005HS, without point of operation guard.  f. In the southeast area of the shop, an employee was exposed to a struck-by hazard when operating a Duro Dyne (brand), Mach II Pinspotter without point of operation guard.

29 CFR  1904.29(b)(3):Each recordable injury or illness was not entered on the OSHA 300 Log and/or an accident report (OSHA Form 301 or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.  On or about January 17, 2023, an employee sustained a laceration to his fingers on the Hypertherm (brand) Jet Liner II waterjet cutting system, the injury was not recorded on the OSHA 300 Log and/or an accident report (OSHA Form 301 or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.

29 CFR  1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.
