29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Jobsite:  On or about April 5, 2023, an employee fell from the roof while installing a new asphalt shingle roof.  The employee fell about 30 feet and landed onto a concrete pad.   The employee was wearing a rock climbing or seat  type harness and was not properly tied off.  In the fall the employee suffered a fractured spine and a broken right arm.

29 CFR  1926.502(d)(16)(iii):  Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  (a)  Jobsite:  On about April 5, 2023, an employee was replacing an asphalt shingle roof when he fell about 30 feet to a concrete pad.  The rigging was not rigged to limit the employees fall to a maximum of 6 feet.

29 CFR  1926.502(d)(16)(iv):  Personal fall arrest systems, when stopping a fall, did not bring an employee to a complete stop and/or and did not limit maximum deceleration distance to 3.5 feet (1.07 m):  (a)  Jobsite:  On or about April 5, 2023, the personal fall arrest system did not bring an employee who fell from a roof to a complete stop nor did it limit his maximum deceleration distance to 3.5 feet.  An employee fett about 30 feet from the roof and landed on a concrete pad.

29 CFR  1926.502(d)(16)(v): Personal fall arrest systems, when stopping a fall, did not have sufficient strength to withstand twice the potential impact energy of an employee free falling a distance of 6 feet (1.8 m) or the free fall distance permitted whichever is less:  (a)  Jobsite:  On or about April 5, 2023, an employee fell about 30 feet from a residential roof that he was replacing asphalt shingles on.  The fall protection that the employee was using did not limit his fall and he fell approximately 30 feet to a concrete pad on the ground.   The employee suffered a broken right arm and spinal fractures as a result of hitting the concrete pad at the end of his fall..

29 CFR  1926.503(c)(3):  When the employer has reason to believe that any affected employee who has already been trained does not  have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee. \:  (a)  Jobsite:  On or about April 5, 2023, the employer needs to retrain its employees in the proper use and installation of personal fall protection when working from a residential roof.
