29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:   a) On April 7, 2023, employees were exposed to eye injury while operating a power tools  without eye protection during masonry work.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(e)(1): Safe access was not provided to scaffold platforms when the platforms were more than 2 feet above or below the point of access:  a) On April 7, 2023, Employees accessed a tubular welded framed scaffold platform while climbing the ends of the scaffold frames, which were not designed for such use.   b) On April 7, 2023, Employees accessed a tubular welded framed scaffold platform while climbing the cross braces of the scaffold frames, which were not designed for such use.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.451(g)(4)(i):Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  a) On April 7, 2023, Employees engaged in masonry work were not protected from the hazard of falling while working from a tubular welded frame scaffold platform elevated approximately 18.8 feet above the ground.
