29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on flat roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, warning line system, or a safety monitoring system. a) Roof level: An employee was exposed to a fall hazard of approximately 20 while talking on the phone and was walking on a flat roof, on or about 4/10/23. Employee was not utilizing any means of fall protection. b) loading dock extended roof: An employee was exposed to a fall hazard of approximately 14 feet while working on a flat roof without the use of warning lines, personal fall arrest system or other means of fall protection, on or about 4/10/23

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) Extended roof over loading dock: The employer did not provide a ladder or other safe means of access for an employee working on a n extended roof that was 4 feet below the main roof, 4/10/23. Employee access the lower roof by jumping and pulling himself up to get to the main roof, owner was on site.
