Energy control procedures including but not limited to the Sawmill Chip System did not address the hazards and specify the measures for controlling the hazards. (a) Written procedures did not specifically outline the steps for shutting down the equipment. (b) Written procedures did not specifically outline the steps for applying the energy isolating devices to all energy sources and did not identify the type and magnitude of the energy sources. (c) Written procedures did not specifically outline the steps for release of any stored energy, such as but not limited to hydraulic and mechanical energy.
