On or about 04/11/23 the employee did not have the proper scaffold training before using or working from a scaffold.

29 CFR 1904.39(a)(2):The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. Worksite: 1620 Bedford St. Fall River, MA. New Fall River, MA water department building. Employee was injured when he fell from a ladder placed on top of a baker scaffold platform. The employee fell 12 feet to a concrete floor below and sustained injuries. The employee was hospitalized and the employer did not report to OSHA.
