29 CFR 1910.137(c)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and/or the maximum intervals between tests were not conducted in accordance with Table I-4 and Table I-5. a) Maintenance Shop - On or about April 11, 2023, two sets of electrical insulating gloves available for employee use were purchased in 2009 and have not been tested since. Multiple employees regularly work on electrical components and are exposed to "live" electrical hazards.

29 CFR 1910.335(a)(1)(i):Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: a) Production Floor - On or about April 6, 2023, employee(s) perform electrical work within the restricted approach boundary of an energized electrical installations but are not provided electrical protective equipment that would prevent body parts from making contact with energized electrical components and protection from arc flash.
